Signature Fields
- Signature
- Placeholder for a signer to input their legal electronic signature on a document
- Initials
- Placeholder for a signer to input their legal electronic initials on a document
Auto-Fill Fields
- Date
- Automatically inputs the current date when a form is filled/signed. Here is how to control the date format.
Standard Fields
- Textbox
- Placeholder for a signer to type information onto a form
AND - Particular information can automatically populate into the document from the Talent Profile each time you send a Signature Request. Textbox Fields that you desire to have pre-populated information, should be Assigned To the Sender and appear in purple in the Assigned To section and throughout the template.
- You are able to choose the information you’d like to pre-populate from the Talent Profile by selecting from the options in the menu in Settings.

- Placeholder for a signer to place a checkmark onto a document. You can group checkmarks together and select how many boxes must be checked.
- Dropdown
- Placeholder for a signer to select an option from a dropdown list to be populated onto the document. Choices that appear in the dropdown list are established by the template creator when the dropdown field is added to the template.
- Radio Groups
- Placeholder for a signer to choose between two or more options such as choosing A, B, C, or D on an exam.
For Example:
When requesting a Skills Checklist from a healthcare professional, the document may ask for the healthcare professional to rate particular skills on a scale of 1-5. Radio Groups can be used in this formatting.

Field Type Settings
When adding a field to a template, it is important to note the settings applied to the field. Settings vary by field type.
- Assigned To
- Indicator of who will complete the field when the template is used in a signature request
- Talent
- Assigning a field to Talent means that the healthcare professional will complete the field. Fields assigned to Talent appear green in the Assigned To section and throughout the template.
- Sender
- Assigning a field to Sender should only be used when using a textbox field that you’ve selected to pre-populate information. The Sender in this case is the Kamana Platform. This means that when the document is sent, the Kamana platform is going to populate merge field data on the document. Learn more about AutoFill Fields.
- Me(Now)
- Assigning a field to Me(Now)means that you will complete the field during the template creation process. Use this when permanently adding an already completed field to the document, such as your agency’s name.
- Required Field
- When using a textbox, checkbox, dropdown, or radio group you can choose for the field to be required or not by the signer. Signatures, Initials, and Dates are all required by default.
- Pre-Filled Information
- When using a textbox field, you can automatically populate certain information into the document from the Talent Profile each time you send a Signature Request.

- When using a textbox field, you can automatically populate certain information into the document from the Talent Profile each time you send a Signature Request.
- Placeholder Text
- You are able to control what is displayed in the textbox field prior to it being filled by a signer. You are not required to enter placeholder text. However, doing so will ensure clarity to the signer.
For Example:
For an optional ‘Former Phone Number’ field, a suggested placeholder text would be “Enter Former Phone Number, if applicable”

- Fonts and Font Sizes
- Most field types allow fonts and font sizes to be customized
- Validations
- Available for use with Textbox Fields. Validation enables you to choose what type of information is allowed to be entered into a field by a signer.
- Validation examples include numbers only, letters only,
- For Example:
- When asking for an email address, selecting the Email Validation will require the signer to enter an @ symbol when filling the field
- Checkbox Settings
- Checkboxes can be checked or not by default
- If using one or more checkbox that is related to the same question or section of the form, the boxes are able to be grouped. This will allow a minimum and maximum number of checkboxes that are able to be checked per group.
- Radio Group Settings
- Radio buttons are stacked vertically by default but can be rearranged to be horizontal and can be spaced out as desired.
- To rearrange a radio group, after adding a radio group to the form, click on a single radio button and drag it as desired.
- To resize a radio group or move an entire group at once, click and drag a box around the radio group for the resize and move options to appear.
- Field Names
- You are able to add an optional Field Name to any field. They are generated automatically when the field is added to the form. Field names are not seen by the signer but can serve as a reminder to you of what the field is.
